Sick Leave Policy Template

This template provides a basic structure for your sick leave policy.

Purpose

Chances are good you’ll get sick sometime during your tenure with us. Whether it's a short stint or something that requires a little more medical attention, this policy walks you through taking the time you need to focus on your health.

This policy applies to all [full-time and part-time] employees who have been employed for at least [90 days].

What is Sick Leave?

Sick leave is paid time off that is set aside for reasons related to your health (physical, mental, and emotional) or the health of an immediate family member. It is separate from our vacation policy and does not require employees to request time off in advance.

Our Sick Leave Policy

Our sick leave policy allows employees to take up to [5] days of sick leave every year. These days are granted upfront at the start of every calendar year (no accrual).

And employees may roll over up to [40] hours of unused sick leave at the end of the calendar year.

Caring for a family member

If an immediate family member falls ill and requires intensive care, you can use your sick time to look after their health.

Extended sick leave

If you need to take more than [5] consecutive days off for an illness or accident, you may be able to tap into your vacation days. Meaning, you can piggyback up to [10] additional vacation days to supplement your sick leave (taking care of you, your family, or to mourn a loss).

Please contact [HR] as soon as possible if you know you’ll be taking sick leave for longer than [5] days.

How to Request Sick Leave

Please submit sick leave requests via [our Human Resources Information System (HRIS)]. And contact your manager as soon as you realize you will be taking sick leave. Please communicate when you will return to work (an estimate is okay) and if you have any projects that need coverage.

While on sick leave for multiple days, please check-in daily, if possible. Or designate a 3rd-party to check-in, so we know how you are doing and when you will return to work.

In all cases, remain communicative with HR, and modifications to our sick leave policy may be considered in unique circumstances. In these cases, we may require a doctor's note or for you to fill out a sick leave form. In these situations, [HR] will reach out to guide you through the process.
